Use these forms to allow your campers to sign-up for Sports and Activities prior to your arrival at BPYC. These forms will also be available during Registration. All completed forms must be submitted prior to 4pm on Monday of camp.

Camp Participant Roster

Head Counselors: To make things run smooth during Registration, please use this form to keep track of the camp participants from your church. Please include a coy of this form with your pre-registration materials.  Keep this form updated as your registration changes and bring a final copy with you on Monday morning as you prepare to travel to Barbourville. This way you will know exactly how many participants you are bringing and it will make the registration process faster.
